HACCP+ Documents: Standard, Audit Forms and Checklist

The HACCP+ documents are the normative standard (version 01/2026), earlier English editions, the system presentation and the audit toolkit: the audit plan (FA002), the attendance list (FA001), the auditor’s declaration of independence (FA003), the audit report template and the checklist workbook. All are free to download.

HACCP+ Standard — structure and table of contents, version 001 (2021)

English edition of the first published version of the standard. It sets out the complete chapter structure (0–8) and indicates which recognised systems may demonstrate compliance with each chapter, e.g. ISO 9001 for chapter 3, BRC/IFS/ISO 22000/FSSC 22000 for chapter 4, ISO 14001, ISO 50001 and ISO 37120 for chapter 7. Useful as an English overview; the 2026 draft is the reference for requirements.

Audit forms

FA002 — Audit plan

Form agreed between the auditor and the organisation before the audit: company data, audit language, scope, audit type (first audit, surveillance, recertification), multi-site structure and sampling, number of shifts, audit location, date and duration, followed by an hour-by-hour agenda linking each audit area to the relevant chapter of HACCP+, legal requirements or Codex Alimentarius.

FA001 — Audit attendance list

Records the names and signatures of the organisation’s staff present at the opening and closing meetings of the audit. It is part of the audit evidence and confirms that management was informed of the audit findings.

FA003 — Auditor declaration of independence and confidentiality (GDPR)

Signed by the auditor before every audit. The auditor declares that they have not provided consultancy, internal audits or company-specific training to the audited organisation (or related companies) in the two years before the audit and will not do so for two years after it, that there is no conflict of interest, and that audit information and documents will be kept confidential.

Audit report and checklist

HACCP+ audit report and checklist (workbook)

The working tool of the HACCP+ audit. Sheet “raport”: cover page with organisation data, audit type, lead auditor, result in % and decision rules (100 % – no non-conformities; 70–99 % – corrective actions to be verified; below 70 % or KO – report sent to Belevo and an additional inspection required). Sheet “dane”: organisation profile and scope by sector. Sheet “lista”: the checklist of requirements graded A / B / C / D / N/A with comments. Sheet “niezgodnosci”: register of non-conformities, agreed corrective actions and deadlines.

Version control

Each new version of the standard contains a list of changes with the date of entry into force and the transition period for certified organisations. Always use the version in force on the date of the audit. If you are unsure which version applies to you, contact us.
